COSA offers free online Steps to Healthy Living
Chronic Disease Self-Management Program
Did you know: The Centers for Disease Control and Prevention finds that six in ten Americans live with at least one chronic disease and four in ten adults have two or more of these conditions. Illnesses like heart disease, cancer, chronic lung disease, stroke, diabetes and other chronic diseases are the leading causes of death and disability in America, and also a leading driver of health care costs.
If you are age 60 or older and living with a chronic condition, you know the daily challenges that chronic conditions bring, but it does not have to be! Don’t let an ongoing health problem rule your life. Let 2021 be your year to live healthier, feel better, be in control and enjoy life!
The Delaware County Office of Services for the Aging (COSA) is sponsoring the acclaimed, evidence- based Steps to Healthy Living: Chronic Disease Self-Management Program developed at Stanford University to provide participants with self-care tools and life skills to support them in reaching their personal goals to manage their chronic conditions.
The program will meet online once a week for 6-weeks on Tuesdays
February 3, 2021 through March 10, 2021 from 10:00 a.m. to 12:30 p.m.
Don’t miss out. The program is offered FREE and includes your personal copy of the book Living a Healthy Life with Chronic Conditions, through support from COSA and the PA Department of Aging. Space is limited and registration is required.
